Equipment Rental Waiver & Assumption of Risk
By submitting this rental request and renting equipment from CPK RENTALS, I acknowledge that the use of pressure washing equipment, accessories, and related machinery and equipment involves inherent risks, including but not limited to personal injury, property damage, equipment damage, and loss.
I confirm that I am capable of safely operating the rented equipment, or that any person operating the equipment under my responsibility is properly supervised. I agree to use the equipment only for its intended purpose and in accordance with all provided instructions and safety guidelines.
I voluntarily assume full responsibility for any injury, damage, or loss that may occur during the rental period, including damage to the rented equipment itself, regardless of the cause.
I hereby release and hold harmless CPK RENTALS, its owners, employees, and affiliates from any and all claims, demands, liabilities, actions, or causes of action arising from the rental or use of equipment, to the fullest extent permitted by law.
I understand that submitting this form does not guarantee availability. Rental approval, pricing, delivery or pickup, and additional rental terms will be confirmed by CPK RENTALS prior to final approval.
